Description
Orient a supplied bounded research item without a validated nursing scale.
Care and training records use task identifiers, source labels, counts, time intervals, observations, and accounting bases. Documentation support, actual delivery, and clinical quality are different questions. A local record rule or budget should be interpreted according to its explicit inclusion and denominator conventions.
Orient a supplied bounded research item without a validated nursing scale.
When to use this tool
Use Nursing Item Response Polarity when you need this care-record, observation, training, supply, time, or accounting component.
How to use the tool and how it works
- Enter response, lower, upper, higherOriented. Use the units and conventions below.
- Run the tool. Correct any format, range, or cross-field input error before interpreting the output.
- Read oriented. Use the output convention below.
Require lower<=response<=upper. Return oriented=response if higherOriented, otherwise lower+(upper-response).1
| Input field | Meaning and units | Accepted input |
|---|---|---|
| response | Response | Required. Finite number. |
| lower | Lower | Required. Finite number. |
| upper | Upper | Required. Finite number. |
| higherOriented | Higher Oriented | Required. Boolean. |
| Output field | Interpretation | Returned shape |
|---|---|---|
| oriented | Oriented | Required. Finite number. |

} Limitations and assumptions
- Response and bounds need one numeric scale. Reversal does not validate an instrument, convert ordinal data into interval measurements, interpret item wording, or establish a clinical score.
- Inputs must satisfy both the field types and the cross-field conditions above. Invalid input causes an error; a successful false flag or documented placeholder is a different result.
- Numeric calculations use binary floating point unless exact decimal-integer arithmetic is explicitly stated. No hidden measurement tolerance or decimal rounding is added; nonfinite or out-of-contract results are rejected. Numeric signed-zero results are exported as zero.
Alternative or Complementary methods
Use authenticated records and the intended care, inventory, or training protocol for a full assessment. Verify source support, counting rules, time bases, and any contractual or consent conditions independently.
